  • Journey to Better Health: Faith-Based Health Initiative

    Journey to Better Health works to build the capacity of faith-based organizations to improve the health of their congregants. This is accomplished through two strategies: 1) Empowering congregants to manage their chronic disease and 2) Reducing hospitalizations by addressing social determinants of health.

  • Community Care Team

    In June 2016, Healthy Howard’s Community Care Team (CCT) transitioned from a program of Healthy Howard to a program of Howard County General Hospital (HCGH). The CCT model remains unchanged and continues to provide home-based care management to frequently hospitalized Howard County residents. We look forward to improving transitions from hospital to home.   • Advanced Primary Care Program

    Healthy Howard’s Advanced Primary Care Program worked to improve primary care delivery for Howard County residents by helping providers work toward Patient Centered Medical Home (PCMH) standards. This program will merge with HCGH’s Primary Care Advisory Council in FY17.

  • Re-entry Services

    As of July 1st, 2016, Howard County Department of Corrections is administering this program. This program began in August 2012 when the Howard County Detention Center was selected as one of six jurisdictions from across the country to receive a grant from the Urban Institute and the National Institute of Corrections. When inmates from Howard County Department of Corrections facilities are released, Healthy Howard helped ensure that they had continued access to health care through the Re-entry Health Services program.
